Getting Your Cases Placed
Mailed applications may be accompanied by an initial premium check for the first modal premium due.
Fidelity Life will draw the first premium via credit card or EFT if no check is received provided the bank or credit card information is included on the application (if initial draft from bank or credit card is not desired, please note on application).
